Integrated "Smart" Biopsy Guidance
By 2026, "blind" biopsies using only fluoroscopy are being phased out in academic centers. Instead, surgeons use Multimodality Image Fusion. This technology overlays the patient's pre-operative Cardiac MRI (CMR) or PET scan onto the live X-ray screen. This allows the surgeon to target "hot zones"—areas of the heart showing active inflammation or scarring—rather than taking random samples.
Additionally, Intracardiac Echocardiography (ICE) is frequently used during the procedure. An ultrasound probe is placed inside the heart to provide a real-time, high-definition view of the bioptome's position. This ensures the jaws are firmly against the interventricular septum before a sample is taken, drastically reducing the risk of puncturing the thinner free walls of the heart.
